If you were confused by the first Metal Gear Rising: Revengeance trailer then you're still going to be confused for the second trailer that was recently released.
The new trailer still has Jack in some procedure and it seems like he might be being brainwashed. Either way this is obviously him between Metal Gear Solid 2 and Metal Gear Solid 4.
